Appointments & Scheduling
We schedule all visits by appointment only on a first-come-first-serve basis. Please schedule your appointments in advance either in person or by calling our clinic at 972-401-0300. If you are unable to attend an appointment, we appreciate at least 24 hours notice.
What to Bring to Your First Visit
- Prescription for physical therapy from
your referring physician
- Government issued, photo ID
(such as a driver’s license)
- Insurance card(s)
- Appropriate attire (workout clothes)
- Patients who are minors (under 18 years of age) must be accompanied by a parent or guardian for their first visit
- Patient forms, which can be downloaded from this site in PDF format. Print and fill out the forms completely at your convenience and then bring them to your first office visit. We understand your time is valuable.
